How to use it

  1. Fill in the insured person’s part: your CNSS registration number, your name, and the patient if it is a family member.
  2. Ask the doctor to fill in, sign and stamp their part, and the pharmacist or lab to fill in theirs and stick on the medicine labels.
  3. Attach the prescription and the receipts, and keep a copy of everything.
  4. Hand the file in at a CNSS agency.
